Jared Romer Cruz is a Board Certified Family Nurse Practitioner. He graduated with his Bachelor’s of Science in Nursing at Chamberlain University in 2017. He spent his nursing career at Northwest Community Hospital as a heart and renal failure nurse and became a Progressive Care Certified Nurse through the American Association of Critical-Care Nurses. Jared went on to complete his Master’s of Science in Nursing at Chamberlain University in 2020. He obtained his Nurse Practitioner Board Certification through the American Nurses Credentialing Center. His areas of interest include Primary Care, Emergency Medicine, and Cardiology.
Ambar Cantu is a patient- centered advanced practice nurse specializing in Family Medicine providing preventative care and treatment for chronic conditions. “I make every effort to empower my patients through education, and we work together, as a team, to improve their health and wellness. It is a wonderful thing when a career and a passion are one and the same”, Ambar says.
Ambar earned her Bachelor’s degree in nursing from Chicago State University. After more than 9 years as a registered nurse, in a variety of settings including mental health and cardiovascular, she earned her Master’s degree from Chamberlain University, to become a family nurse practitioner.
Outside the office, Ambar enjoys spending time with her family, which include her husband, son, two dogs and two cats. She loves camping in the wilderness, reading, and listening to music in her free time.
Linta Midhun, FNP-BC is a board-certified Family Nurse Practitioner, with 10-year experience in nursing role. Adept at critical care and management with training in patient education and leadership. Linta is dedicated to give friendly care to her patient and their family. She works closely with a team of professional providers and provide care with compassionate.
Linta received her Bachelor of Science in Nursing from Rajiv Gandhi University, India and Master of Science in Advanced Practitioner from North Park University, Chicago. She became board Certified as Family Nurse Practitioner (FNP) with The American Nurses Credentialing Center(ANCC). She is committed to the evidence – base quality outcome of all patient in this practice.
Mari Collins, is an MSN, APRN, AGACNP-BC who treats young adult to geriatric patients with acute, critical or chronic illness and injury. Mari works at various clinics in the Chicago suburbs that provide services to diverse populations ranging from urgent care and primary acute and chronic illness management, to COVID-19 testing, as well as a vascular clinic that specializes in amputation prevention.
Mari believes that healthcare should be accessible to everyone, and has spent her nursing career practicing in underserved communities, treating everyone with dignity and respect.
Mari is passionate about educating her patients, and planning care in a holistic collaborative manner, to achieve their best level of health.
Ms. Collins started her healthcare path in the military where she was an Honor graduate from Walter Reed Army Medical Center. She then received the award of Best and Brightest students from Purdue University, and finally, while obtaining her Master’s degree, gained recognition as an outstanding nurse from the International Nursing Association.
She is also an instructor for ESCI institute, and in her spare time rescues dogs, loves to travel, read, and attend live music venues.
